For a limited time, the Farmingville Hills Chamber of Commerce is running a EARLY BIRD DISCOUNT. But hurry, this offer won’t last long. You must fill out the application below and remit payment prior to 11:59pm on May 8, 2019.

NOTE:
1. All exhibitors MUST know their New York State Tax Certificate of Authority number when filling out the application below. If you do not have a New York State Tax Certificate, here’s how to obtain one.
2. All exhibitors MUST display their New York State Tax Certificate of Authority number the day of the event.
3. All food vendors MUST obtain a permit from the Suffolk County Department of Health.
